LSSI is hiring a full-timeLicensed Practical Nurse.To provide home and community-based services focused on health promotion and disease monitoring to Colbert and Williams CST, CST, and CCM clients. Coordinate treatment with psychiatry/APN/case managers/medical physicians/MCOs/etc.

Essential Functions:
  • Provide health promotion education to clients and guidance to staff.
  • Medically screen clients to provide disease management in accordance with the psychiatrist/APN/medical physicians.
  • Provide direct nursing services to clients for health history, weight BMI, etc. in their homes and in the community.
  • Coordinate health information exchange with psychiatrist/APN/medical physicians/MCOs in accordance with privacy laws.
  • Administer injections and coordinate medications/refills with pharmacies.
  • Maintain documentation in accordance with agency and regulatory requirements.
  • Transportation of clients in personal vehicle and agency vehicle is required.
  • Other duties as assigned.
  • Establish medical treatment plans for clients with health risks/conditions.
  • Connect clients to health home, as needed.
  • Meet financial productivity requirements and complete all documentation per Rule 140/132 requirements and Consent Decree requirements.
  • Demonstrate proficiency in Electronic Health Record.
Position Qualifications:
  • Minimum of Associates degree in nursing is required, along with proven nursing skills.
  • LPN
  • Associates in Nursing is required.
  • Must be willing to work with clients in the home/community.
  • Ability to engage and motivate persons with mental illness.
  • Basic computer skills.
  • Must have excellent written and verbal skills.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
  • Must have a valid Illinois driver's license in good standing required and insurance.
