Breast Imaging Navigator - Methodist

Position Summary:


This position oversees routine and specialized diagnostic mammography procedures in Mammography for patients of all ages in order to confirm or deny required treatment. This position is to advance the physical and emotional care of women diagnosed with breast cancer or other breast health issues. As a multidisciplinary team member, the incumbent is responsible for assisting women and their network of support through the various diagnostic modalities related to breast health issues. Patients will be monitored via the EPIC EMR, telephone calls, email reminders, and internal registry processes to ensure compliance with HIPAA and other accreditation requirements.


Qualifications:


EDUCATIONAL REQUIREMENTS
Associate's Degree or Experience in Lieu of Education - Oncology Care Navigation and/or Ambulatory Care Navigation

SKILLS AND KNOWLEDGE
Strong communication and customer service skills. Self-motivated and positive attitude. Self-directed with the ability to work independently. Strong organizational skills. Demonstrated leadership abilities.


Essential Functions:

  • Document in a timely manner required information into the electronic record.
  • Maintains professional interaction with patients and staff.
  • Make reminder calls to decrease no shows and reschedule noshows as needed.
  • Assists in expediting and coordinating diagnostic imaging, as needed.
  • Find creative solutions to patient barriers of care.
  • Maintains patient confidentiality and HIPAA compliance.
  • Serves as a liaison between the patient, nurse navigator, physician, radiologist, and lead mammo tech.
  • Supports and assists with all patient education needs.
  • Provides coordinated care to patients by utilizing an interdisciplinary approach.
  • Communicate with the patient's other physicians as needed.
  • Review medical records to determine if follow up care or imaging was completed and track results.
  • Educate patients and the families regarding diagnostic imaging modalities and their differences.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Outreach to patients due for preventive health services or chronic disease management services.
  • Collaborates with a team of healthcare professionals to share expertise and knowledge.
  • Acts as liaison for grants pertaining to breast health.
