The Community Health RN represents Carilion in outreach activities coordinated with community partners and other departments of Carilion Clinic. Provides evidence-based health education and performs health screenings in a variety of community settings served by Carilion Clinic. Collaborates with other health care professionals to provide evidence-based guidance to members of the community in support of health promotion and illness/injury prevention. Supervises LPNs and non-licensed Outreach staff in execution of clinical outreach duties. Supports Carilion's hallmarks of service excellence and quality for all job responsibilities.

Minimum Qualifications

Education: Graduate of an accredited school of nursing is required. Bachelor's degree is preferred.

Experience: Three (3) years of professional experience required, including demonstration of basic nursing clinical competencies and patient teaching. Experience in public health or occupational health is desirable. Background in public health preferred

Licensure, certification, and/or registration: Licensed as a Registered Nurse by Virginia Board of Nursing or other body that is recognized under Compact Licensing agreement for practice in the Commonwealth of Virginia; Basic Life Support, valid Virginia Driver's License

Other Minimum Qualifications: Must be service oriented and possess excellent verbal and written communication skills. Requires the ability to educate clients of any age or SES level on health issues and injury prevention topics; demonstrate knowledge and skills to provide educational components, knowledge principles of growth and life span development, assesses specific participant information to identify risk factors. Proficient in Microsoft Office applications and other basic technology such as email and electronic documentation is highly desirable. Must be capable of working independently with minimal supervision and possess decision-making skills based on expertise and professional judgement. Must be available to work flexible hours. Must provide own transportation and travel to offsite work areas as needed. Willingness to travel to other Carilion Clinic service areas is considered an asset.
