We have an amazing opportunity for a Maternal Outreach Nurse in our First Connections Program. RN willplan and implementtheassessment and coordination of care with women prenatally,childrenand their families from intake through referral planning andtransition. Coordination of care is provided in collaborationwith families, birthing hospitals and other community referral sources. Meeting with families to provide screening and assessment in determining appropriate referral plans and documentingprogress in referral engagement.Educate on various topics related to maternal-child health and development.Minimum of 1-3 years of direct service experience with young children and their families and 3 years of public health and community development experience.

Education and Licensing Requirements: RI Registered Nurse license required to work in the state.
