We focus on Integrity, Compassion, Relationships, Innovation and Performance as we empower people to achieve better health and well-being.Primary Responsibilities:Create a positive experience and relationship with the memberKeep the member out of the hospital by supporting regular visits to their primary physicianKeep member actively engaged with their primary physicianSupport the member to ensure pick-up of their RxProactively engage the member to manage their careProvide member educationSupport transitions of careHelp to keep members compliant with their care plansPartner with care team (community, providers, internal staff)Knowledge and continued learning of community cultures and valuesMay conduct HRA assessments if needed

Required Qualifications:High School Diploma / GEDMust live in Johnson County, KS or Wyandotte County, KS or Leavenworth County, KS.Ability to travel up to 40% of the time for field based work (radius includes 30 miles from the staff members home zip code)Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated areaMust have resided within the community for 2+ yearsKnowledge of culture and values of communityFamiliarity with the resources available in the communityField based experienceSchedule is Mon - Fri 8:00am to 5:00pm with flexibility to adjust schedule based on member's needsPreferred Qualification:Bachelors Degree (or higher) in social work and / or health care administrationLicensed practical nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical AssistantBilingual fluency in Spanish and EnglishCHW accreditationExperience working in managed careKnowledge of Medicaid populationCareers with Optum.
